Community Case Managers supported people living with HIV and the Health Promotion Case Management team supported people with current, ongoing risk factors for becoming HIV-positive. Across both programs, our dedicated team works to coordinate care, provide mental health support, address substance use concerns and harm reduction needs, find and maintain housing and employment, immigration and citizenship needs, and support navigating Ministry systems such as disability and income assistance.

AIDS Vancouver's Case Managers work to meet your unique needs through thorough assessment and care planning. Our immediate focus is on food, shelter, income, and emotional support, but we may also work with you on:

Connecting with a Case Manager

Our communities are welcome to connect with a case manager anytime. You do not need a referral.

  • if you are living with HIV, give us a call or drop in! You can call us at 604-893-2201 or drop in to our office on the 4th floor at 1101 Seymour Street between the hours of 10:00am and 4:00pm, Monday through Thursday. We are not open on statutory holidays or Easter Monday.
